Blackthorn City is located in northeast Johto. Pokémon Trainers often come to Blackthorn City in order to win the Rising Badge from Clair, the Blackthorn Gym Leader, who specializes in Dragon-type Pokémon. Many famous Dragon-type Trainers, including Clair and Lance, come from Blackthorn City. According to a resident in Blackthorn, most Trainers in the city end up being Dragon-type specialists. This could be because of the Dragon's Den in the area and because the local Gym Leader is a Dragon-type Trainer.

So to prove yourself, Clair request the player to take on the Dragon User Challenge, in which the player much go deep into the Dragon Den to retrieve the Dragon Fang. Once the player retrieves the Dragon Fang, Clair will acknowledge her loss and hand over the Rising Badge, encouraging the player to take on the Pokémon League.

A clan of masters of Dragon-type Pokémon have lived in the city for generations. The masters built the Dragon's Den, a cavern with an underground lake that contains a shrine to Dragon Pokémon, as well as the elders of the Dragon clan. The entrance is located behind the Gym, across a small pond, while the main cavern seems to be located underneath the city itself.
The Dragon's Den appeared in Fangs For Nothin' (episode 252), where it was reduced to a series of tunnels leading to the Dragon Holy Land. The Dragon Holy Land takes over for the Dragon's Den's other roles, including the location of the Dragon Shrine as well as of the Dragon Fang.
